Assertive Community Treatment
Assertive Community Treatment a is team-based, client-centered, intensive mental health service that adheres to evidence-based practice designed to serve individuals with severe mental illness. Assertive Community Treatment (ACT) provides long term needed treatment, rehabilitation, and support services to identified individuals with severe and persistent mental illness especially those who have severe symptoms that are not effectively remedied by available treatments or who because of reasons related to their mental illness resist or avoid involvement with mental health services in the community. ACT services are offered outside of clinic, hospital, or program office settings for individuals who are best served in the community. ACT services include psychiatric assessment, counseling, medication management, and care coordination activities through a designated multidisciplinary team of mental health professionals.

Individuals qualifying for MHSS must demonstrate a clinical necessity for the service arising from a condition due to mental, behavioral, or emotional illness that results in significant functional impairments in major life activities.

An intensive outpatient program (IOP) is a structured treatment program that helps people manage mental health issues or substance use disorders without requiring 24-hour supervision or medical detoxification. IOPs are more intensive than standard outpatient programs but less intensive than inpatient rehab.

An Assertive Community Treatment (ACT) team consists of a community-based group of medical, behavioral health and rehabilitation professionals who use a team approach to meet the needs of an individual with severe and persistent mental illness.
